In this role, you will assess learning needs, develop individualized education plans, and provide in-person and virtual coaching on medications, devices, lifestyle modification, and symptom monitoring. You will collaborate closely with physicians, nurses, case managers, and pharmacists to align care plans, promote adherence, reduce hospital readmissions, and improve quality metrics.
